Cancellation Policy

1. If you cancel your sleep study booking before Aer's staff sends the sleep study kit to you, you may be eligible for a refund of the admin fee minus the booking fee of $45. This fee is an administration fee required to pay the GP for the referral given. Should you be required to pay this fee, we will provide you with a copy of the GP referral. 2. If you expect to receive any refund, you need to tell us no later than three business days before your chosen appointment date that you wish to cancel; otherwise, your payment may be forfeited in full for the $199.00 you have paid. 3. If you cancel your sleep study booking after Aer's staff has sent you the sleep study kit, the penalty fees noted below will apply. 4. The Aer Healthcare home sleep test is partly funded under Medicare for people 18 years old and over via item 12250 of the MBS schedule. We also ask for a $199 fee for administration and shipping and handling. 5. The allowance for item 12250 is available only once in any 12 months. This is a one-night test; you will not perform the test on more than one night 6. Your out-of-pocket expense for the Home Sleep Test is $199. 7. By signing this document, you declare that you have not had a home sleep test performed in the last 12 months under Medicare item number 12250 and that your Medicare card details are accurate and have not expired. 8. If, for any reason, our attempts to retrieve the Medicare payment (the assignment of the right to benefit to the practitioner who has rendered the service) do not work for your Medicare claim for MBS 12250, we will charge you the fee for what Medicare would have covered ($324.70); we will provide the necessary documentation so that you can submit a Medicare claim for MBS 12250 yourself.
